How To Register
We register patients living in Lewisham, Blackheath, Lee and the surrounding area. Please enquire with reception to see if your address falls within our catchment area.
Patients wishing to register with this practice will be asked to bring an NHS card (if possible) and details of their previous practice. You will then be asked to complete a registration form and you will need to bring proof of address (ideally 2 utility bills) and proof of identity (passport or driving licence) with you. An appointment will then be made for you to see a nurse or health care assistant for a health check. Having had the health check, you will then be fully registered and able to consult with a doctor.
When you register at Belmont Hill Surgery, you are registering as a patient of the practice, rather than with a specific doctor. If you wish, you may express a preference to see a specific doctor and we will try to accommodate your wish. However this may not always be possible but we can assure you that, with excellent liaison between our clinical staff and comprehensive computerised medical records, any of the doctors will be able to help you when you visit the surgery.
Disabled patient facilities
Disabled patient facilities are also available at this practice, car space, ramp, toilet etc).
Change of Personal details
Once registered at the surgery, should you change your name, address or telephone number at any time, please let us know by contact reception or writing to the surgery. It is also helpful to let us know a change of mobile number as soon as possible.
If you move outside the practice area you will be asked to find another doctor nearer to where you are living.
